29 CFR 1910.147(c)(8):Lockout or tagout was not performed only by the authorized employees who are performing the servicing or maintenance: (a) Facility: On or about July 11, 2023, the operator of the mold machine locked out the mold machine. A different employee (a maintenance man) then went into the mold machine to troubleshooting the mold machine. The maintenance man did not have his lock on the power source to the mold machine. In addition, the key to the lock that was used to lock out the mold machine (by the mold machine operator) was kept in an open desk drawer to the mold machine.
