On or about July 11, 2023, at a worksite located at 371 Trinity Way E., Lot 3596, Newark, Ohio 43055: The employer did not ensure that the employees framing the second story of a residential new build house were protected from falling approximately 11 feet, 6 inches to the ground.

On or about July 11, 2023, at a worksite located at 371 Trinity Way E., Lot 3596, Newark, Ohio 43055: The employer did not ensure that the employees did not use the top or top step of stepladders. Employees were standing on the top and top step of a 6-foot stepladder, thereby exposed to a 6-foot fall hazard.
