On or about July 11, 2023, at a worksite located at 109 Natalie Lane, Lot 372, Pataskala, Ohio 43062: The employer did not ensure that the employees framing the second story roof of a residential new build house were protected from falling approximately 18 feet to the ground.  b. On or about July 11, 2023, at a worksite located at 109 Natalie Lane, Lot 372, Pataskala, Ohio 43062: The employer did not ensure that the employees working adjacent to the unguarded window openings of the second story were protected from falling approximately 18 feet to the ground.

On or about July 11, 2023, at a worksite located at 109 Natalie Lane, Lot 372, Pataskala, Ohio 43062: The employer did not ensure that the employees did not use the top or top step of stepladders. An employee was standing on the top of a 6-foot stepladder, thereby exposed to a 6-foot fall hazard.
