29 CFR 1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access.   a- 151 N Franklin Turnpike, Ho-Ho-Kus NJ 07423(Residential Roof): CSHO observed employees engaged in roofing activities on a 7 on 12 pitched roof using an extension ladder for access and egress and exposed to fall hazards of 11ft to the asphalt below as the ladder side rails did not extend 3ft above the upper landing surface. Condition observed on or about 07/12/2023.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b)(1)  a- 151 N Franklin Turnpike, Ho-Ho-Kus NJ 07423(Residential Roof): CSHO observed employees engaged in roofing activities on a 7 on 12 pitched roof in which a personal fall arrest harnesses were worn but not secured via an anchorage point exposing the employees to fall hazards of approximately 18ft to the asphalt below. Observed on or about 07/12/2023.
