29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system. 4 Hazelnut Drive, Farmington, Connecticut Chimney repair project: On or about July 11, 2023, and at times prior thereto, employees were exposed to a fall hazard of approximately 10 feet while performing chimney repair activities on a 10/12 pitch roof without fall protection.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: 4 Hazelnut Drive, Farmington, Connecticut Chimney repair project: On or about July 11, 2023, and at times prior thereto, employees were using the following extension ladder to access the roof: A. The ladder in use extended to .5 feet not meeting the minimum 3 feet past the landing exposing them to a fall hazard of approximately 10 feet.
