29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use. Reference: ANSI Z358.1 Emergency Eyewash and Shower Standard 5.4.4 The eyewash unit shall be located on the same level as the hazard and the path of travel shall be free of obstructions that may inhibit the immediate use of the equipment. On or before July 12, 2023, employees handled corrosive chemical(s) such as Kay Multi-Purpose Disinfectant Cleaner and Ecolab SaniSave No Rinse Cleaner Sanitizer while conducting cleaning and disinfecting tasks. Employer had the following: a) Receiver desk area: a plumbed eyewash was blocked by a black box on the floor, forklift charging cables, receiver desk, and reset items that prevented employees from access for quick drenching and/or flushing for emergency aid. b) Chemical refilling station/area: an eyewash was not provided within the work area for 15-minutes continuous flow for quick drenching and/or flushing for emergency aid. Employees were potentially exposed to eye and skin itchiness, redness, and/or corneal abrasion.

29 CFR 1910.132(e): The employer did not ensure that employees were not using defective or damaged personal protective equipment in the workplace. Hazardous waste storage area: On or before July 12, 2023, employees used personal protective equipment (PPE) including splash goggles during hazardous material cleaning operations. The employer did not ensure the splash goggles had been taken out of service as it had visibility restricted by layer of dirt and dry oil. This condition potentially exposed employees to eyes and/or head injuries.
