29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails were not extended at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access; or, when such an extension was not possible because of the ladder's length, then the ladder was not secured at its top to a rigid support that would not deflect, and a grasping device, such as a grabrail, was not provided to assist employees in mounting and dismounting the ladder: a) Job site 1900 West Railroad Street, Heidelberg, PA: On or about July 12, 2023, during roofing activities, the employer did not ensure that the ladder used by employees to get on/off the roof was properly set up in that it was only extended about 2 1/2 feet above the roof line.
