29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) GBP Roofing LLC located at 5953 Course Dr. Westerville, OH 43081:  On or about July 12, 2023, the employer did not ensure that employees engaged in residential construction activities 6 feet or more above lower level were protected from falls. Roofers were removing/installing shingles on a roof that varied from 11-20 feet without the use of proper fall protection which exposed them to falls of over 20 feet.

29 CFR 1926.20(b)(1): The employer did not initiate and maintain accident prevention programs as necessary to comply with this part:  a) GBP Roofing LLC located at 5953 Course Dr. Westerville, OH 43081:  On or about July 12, 2023, employees were performing roofing work on the roof of a residential home at heights of over 11 feet without fall protection. The employer did not initiate and maintain safety and health programs related to the roofing work being conducted.

29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:  a) GBP Roofing LLC located at 5953 Course Dr. Westerville, OH 43081:  On or about July 12, 2023, the employer did not instruct employees in the recognition and avoidance of unsafe conditions and the procedures to be followed in order to minimize these hazards. Roofers were removing/installing shingles on a residential roof at heights of over 11 feet with no means of fall protection/prevention systems in place.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a. GBP Roofing LLC located at 5953 Course Dr. Westerville, OH 43081:  On or about July 12, 2023, the employer did not ensure that portable ladders used to gain access to the roof extended at least three feet beyond the upper landing that they were using for access.  One extension ladder was used to gain access to roof at the front of the home was positioned approximately 18 inches beyond the upper landing.  The following condition exposed employees to falls over 11 feet.
