29 CFR 1910.147(c)(4)(ii): The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the requirements of section(c)(4)(ii)(A)-(c)(4)(ii)(D) of this standard. On or about July 10, 2023, with regard to an establishment located at 1111 Greenheck Drive, Schofield, WI 54476, the employer did not ensure that the hazardous energy controls procedures for Integrated Blanking System #1, including the Operator Safety Control Zones, provided clear and specific guidance for effective means and methods of hazardous energy control during service and maintenance tasks. Operators utilizing the Operator Safety Control Zone portion of the hazardous energy control plan could still access hazard areas when the methods described in the plan allowed for options that did not control potentially hazardous energy.

On or about July 10, 2023, with regard to an establishment located at 1111 Greenheck Drive, Schofield, WI 54476, the employer did not ensure that a periodic inspection of all energy control procedures had been completed within the previous twelve months to ensure that the procedure was determined to be effective, and the requirements of the standard were being followed.

On or about July 10, 2023, with regard to an establishment located at 1111 Greenheck Drive, Schofield, WI 54476, the employer did not ensure that all employees engaged in service and maintenance tasks associated with the Integrated Blanking Systems were trained in the methods and means necessary for energy isolation and control when entering hazardous areas.

OSH ACT of 1970 Section (5)(a)(1) The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees 29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): On or about July 10, 2023, an employee, engaged in unjamming and alignment activities within the STOPA safety zone of Integrated Blanking System 1, was exposed to unexpected hazardous energy in the form of a moving cart. The alternative safety measures described on the Employer?s Operator Safety Control Zone Map for the STOPA safety zone did not provide enough clear and specific guidance for effective means for controlling that hazardous energy.?
