29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):  On or about June 3, 2024, at an establishment located at 1111 Greenheck Drive in Schofield, Wisconsin, in Department 0330 Power Roof - Small Curb Cell; PRV Fabrication Operators were exposed to hazardous electrical and pneumatic energies while unjamming the automatic guard around the auto curb conduit crimper during the down-stroke of the cycle. The machine was not deenergized prior to attempting to unjam the guard, exposing employees to amputation hazards.  As a result, the remaining applicable energy control elements, involving machine isolation [1910.147(d)(3)], lockout/tagout device application [1910.147(d)(4)], dissipation of residual energy [1910(d)(5)(i)], and verification of isolation [1910.147(d)(6)] were not implemented to protect employees from machine hazards.

29 CFR 1910.212(a)(2): General requirements for machine guards. Guards shall be affixed to the machine where possible and secured elsewhere if for any reason attachment to the machine is not possible. The guard shall be such that it does not offer an accident hazard in itself.   On or about June 3, 2024, at an establishment located at 1111 Greenheck Drive in Schofield, Wisconsin, in Department 0330 Power Roof - Small Curb Cell; PRV Fabrication Operators and Utility employees were exposed to amputation hazards created by the auto curb conduit crimper when the cylindrical guard around the punch had a notch on the backside to allow the automatic feeder arm to load conduit pieces. The notch exposed employees to pinch point hazards created by the shear point between the punch and the guard.
