29 CFR  1926.501(b)(6): Each employee on ramps, runways, and other walkways was not protected from falling 6 feet (1.8 m) or more to lower levels by guardrail systems:  (a) On or about July 12, 2023, an employee was replacing a guardrail to a balcony on the front of a residential building without fall protection. This exposed the employee to a fall hazard of approximately 12.6 feet.

29 CFR  1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  (a) On or about July 12, 2023, an employee was replacing a guardrail to a balcony on the front of a residential building and used an extension ladder to access the work area, which did not extend at least 3 feet above the landing. This exposed the employee to a fall hazard of approximately 12.6 feet.

(a) On or about July 12, 2023, an employee was replacing a guardrail to a balcony on the front of a residential building and used an extension ladder to access the work area, which did not extend at least three (3) feet above the landing. The employer did not provide employees with training on the use, limitations, and how to minimize the hazards associated with ladder use.
