Location: 109 Hopkins Street, Wakefield, MA On 7/11/2023, two workers in the baskets of aerial lifts were being hoisted to work locations on a 3rd floor balcony. Neither was securely attached to the boom or basket of the aerial lift, posing a fall risk.

29 CFR 1926.501(b)(1): Employees on a walking/working surface with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level were not protected from falling by the use of guardrail systems, safety net systems, or properly configured personal fall arrest systems, exposing them to fall hazards. Location: 109 Hopkins Street, Wakefield, MA On 7/11/2023, three workers conducted carpentry work on unguarded edges more than 20 feet above a lower surface without personal fall protection. Instance A: A worker was positioning a 6x6 support beam on a 3rd floor balcony deck, less than 1 foot away from an unprotected edge. Instance B: Two workers were fitting new trim boards on a 3rd floor balcony deck, also less than 1 foot from an unprotected edge.
