29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns.  On or about July 12, 2023, one employee was handing up siding materials from the ground to a second employee working from the roof of a house under construction approximately 10ft high without the use of a protective helmet, exposing the employee to struck by hazards.

29 CFR 1926.102(a)(1): Employees did not use eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.  On or about July 12, 2023, one employee working from the ground using a hand held powered circular saw was cutting pieces of siding without the use of eye protection, exposing employees to the hazard of being struck by flying particles.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system.  On or about July 12, 2023, one employee was working from the roof of a new home under construction up to approximately 10ft high with a 6/12 pitch without the use of a personal fall arrest system, exposing the employee to fall hazards.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access.  On or about July 12, 2023, one employee was using a ladder that was not extended at least 3 feet past the upper landing surface to access a roof up to approximately 10ft high, exposing the employee to fall hazards.
