29 CFR 1910.253(b)(2)(ii): Assigned storage spaces for cylinders was not located away from elevators, stairs, or gangways where cylinders could not be knocked over or damaged by passing or falling objects, or subject to tampering by unauthorized persons: a) Shop area, 2719 Library Rd, Pittsburgh, PA 15234: Observed on or about June 23, 2023, compressed gas cylinders being stored along the back wall of the main shop area were not secured to prevent them from being knocked over. Employees working in the main shop area were exposed to struck-by hazards created by the unsecured cylinders.

29 CFR 1910.253(b)(4)(iii): Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet: a) Shop area, 2719 Library Rd, Pittsburgh, PA 15234: Observed on or about June 23, 2023, oxygen cylinders (UN1072) were stored directly next to acetylene cylinders (UN1001) along a wall in the back of the main shop area. Employees working in the shop were exposed to a fire hazard created by the improper storage of the oxygen and acetylene cylinders.

29 CFR 1904.40(a): The employer did not provide an authorized government representative the records within the four business hours: a) Jobsite, 2719 Library Rd, Pittsburgh, PA 15234: On or about July 13, 2023, the employer did not provide the OSHA compliance officer a copy of the employer's OSHA 300 logs within four business hours. The compliance officer requested a copy of the employer's OSHA 300 logs via an email message to the District Manger on July 13, 2023. A second request was made via an email to the Store Manager on November 17, 2023. The employer did not respond to either request.
