29 CFR  1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch:   Employees engaged in grinding activities using the Heavy-Duty Ball Bearing Omaha Industrial Tool OIT Bench Grinder 10-inch wheel bench grinder in the small maintenance shop were exposed to caught-in hazards in that the work rest opening were not maintained at a maximum of one-eight inch.  The work rest was found to be approximately three-sixteenths inch away from the grinding wheel.

29 CFR  1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch:  Employees engaged in grinding activities using the Heavy-Duty Ball Bearing Omaha Industrial Tool OIT Bench Grinder 10-inch wheel bench grinder in the small maintenance shop were exposed to struck-by hazards in that the tongue guard opening were not maintained at a maximum of one-fourth inch. The distance from the abrasive wheel to the adjustable tongue guard was found to be approximately 5/8 inch.

29 CFR  1910.253(b)(2)(ii):   Cylinders were not stored in areas to ensure that the cylinders would not be knocked over:  Employees engaged in assembling rolls activities in the small maintenance shop were exposed to struck-by hazards in that a full Acetylene cylinder was not secured in a storage area, was not chained up, but laying freely in the middle of the walkway. It was not secured from being knocked over or damaged by passing or falling objects, or subject to tampering by unauthorized persons.

29 CFR  1910.1200(e)(1): The employer did not develop, implement, and maintain at each workplace, a written hazard communication program which described how the criteria specified in 29 CFR 1910.1200 paragraphs (f), (g), and (h) for labels and other forms of warning, safety data sheets would be met:  The employer is failing to protect employees from chemical exposure hazards associated with using and handling chemical in the shop and the mill building area. Employees using hazardous chemicals, including but not limited to: WD-40, grease, diesel, gasoline, hydraulic fluid, and engine oil were exposed to dermal and respiratory hazards due to the employer's written hazard communication program not being updated in accordance with the revised Global Harmonized System (GHS) Hazardous Communications Program. The current program was approved and revised in 2013.
