29 CFR 1926.602(d): Powered industrial truck operator training. Note: The requirements applicable to construction work under this paragraph are identical to those set forth at '1910.178(l) of this chapter.  29 CFR 1910.178(l)(6): The employer did not certify that each operator had been trained and evaluated as required by paragraph (l). The certification did not include the name of the operator, the date of training, the date of the evaluation, and the identity of the person(s) performing the training and evaluation.  A) 21 Washington Ave Carteret, NJ 07008  The employer did not ensure that the operator of the Pettibone T8044 rough terrain forklift had completed training and had the proper documentation of the training. This condition exposed employees to stuck by hazards.   Condition noted on or about 07/13/23
