29 CFR 1926.451(g)(3)(iv): Vertical lifelines, independent support lines, and suspension ropes shall not be attached to each other, nor shall they be attached to or use the same point of anchorage, nor shall they be attached to the same point on the scaffold or personal fall arrest system.  a. Located at the job site at 880 Northstar Flats Drive (Building 3), Sunbury, Ohio: On or about July 12, 2023, and at times prior, two employees worked from a pump jack scaffold installing sofit and fascia utilizing full-body harnesses with lanyards and vertical lifelines that were connected to the same Guardian temper anchor. As a result, employees were exposed to fall hazards of up to approximately 25 feet.

29 CFR 1926.451(b)(3): Except as provided in paragraphs (b)(3)(i) and (ii) of this section, the front edge of all platforms shall not be more than 14 inches (36 cm) from the face of the work, unless guardrail systems are erected along the front edge and/or personal fall arrest systems are used in accordance with paragraph (g) of this section to protect employees from falling.  a. Located at the job site at 880 Northstar Flats Drive (Building 3), Sunbury, Ohio: On or about July 12, 2023, and at times prior, two employees worked from a pump jack scaffold installing sofit and fascia which had its front edge approximately 18 inches from the face of the building.  Employees were thereby exposed to fall hazards of approximately 25 feet.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level shall be protected from falling to that lower level. Paragraphs (g)(1)(i) through (vii) of this section establish the types of fall protection to be provided to the employees on each type of scaffold. Paragraph (g)(2) of this section addresses fall protection for scaffold erectors and dismantlers.  a. Located at the job site at 880 Northstar Flats Drive (Building 3), Sunbury, Ohio: On or about July 12, 2023, and at times prior, two employees worked from a pump jack scaffold at approximately 25 feet from ground level, installing sofit and fascia to the exterior of a new-build three-story apartment building without utilizing fall protection measures. Employees were thereby exposed to fall hazards of approximately 25 feet.
