29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person:  (a)Core Framing, LLC at 5501 - 5503 South Huntwood Avenue, Sioux Falls, SD 57110: On or about and at times prior to July 13, 2023, the exposing employer did not ensure an accident prevention program was developed and implemented for the work environment to include regular inspections of the jobsite, materials, and equipment; conducted by a competent person. Employees were exposed to various fall hazards involving the roof, interior open-sided floors, unguarded window openings, stairwells, scaffolding hazards, inadequate use of a stepladder, and hazards associated with the use of a telehandler while conducting residential framing activities. This condition exposed employees to fall and struck-by hazards. . This condition exposed employees to struck-by hazards associated with unsafe operation of the telehandler. Employees were conducting roof sheathing operations when an employee fell to the concrete floor below and later died from their injuries. This condition exposed employees to fall hazards greater than six feet.  (b)Core Framing, LLC at 5505 - 5507 South Huntwood Avenue, Sioux Falls, SD 57110: On or about and at times prior to July 13, 2023, the exposing employer t failed to ensure that employees conducting framing operations on a multi-level residential twin home were protected from falls. Employees were conducting roof sheathing operations without the use of an adequate fall protection system. This condition exposed employees to fall hazards greater than six feet.  (c)Core Framing, LLC at 5501 - 5503 South Huntwood Avenue, Sioux Falls, SD 57110: On or about and at times prior to July 13, 2023, the exposing employer failed to ensure employees conducting framing operations on a multi-level residential twin home were protected from falls. Employees were   exposed to fall hazards due to second-story unguarded window openings along the west side of the residential property. This condition exposed employees to fall hazards greater than six feet.  (d)Core Framing, LLC at 5505 - 5507 South Huntwood Avenue, Sioux Falls, SD 57110: On or about and at times prior to July 13, 2023, the exposing employer failed to ensure employees conducting framing operations on a multi-level residential twin home were protected from falls. Employees were conducting framing operations and were exposed to fall hazards due to unguarded second-story window openings along the west side of the residential property. This condition exposed employees to fall hazards greater than six feet.  (e)Core Framing, LLC at 5501 - 5503 South Huntwood Avenue, Sioux Falls, SD 57110: On or about and at times prior to July 13, 2023, the exposing employer failed to ensure employees conducting framing operations on a multi-level residential twin home were protected from falls. Employees were exposed to fall hazards due to unguarded open sides on the second floor and inadequate guardrails and falls to the first level within the residential property. This condition exposed employees to fall hazards greater than six feet.  (f)Core Framing, LLC at 5505 - 5507 South Huntwood Avenue, Sioux Falls, SD 57110: On or about and at times prior to July 13, 2023, the exposing employer failed to ensure that employees conducting framing operations on a multi-level residential twin home were protected from falls. Employees were exposed to fall hazards due to unguarded open sides on the second floor and inadequate guardrails within the residential property. This condition exposed employees to fall hazards greater than six feet.
