29 CFR 1910.145(c)(3): Safety instruction signs were not used where there was a need for general instructions and suggestions relative to safety measures: (a) EMBA 245 - The offloading end of the extendo conveyor on the WSA compact top prefeeder, model WSA-TPC-2500X1400, which feeds cardboard boards to the EMBA 245, did not have any safety signs instructing employees about the in-going nip points on the extendo conveyor's belts and pulleys, exposing employees to a caught-in hazard, on or about July 12, 2023. No abatement certification or documentation required.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, in-going nip points, rotating parts, flying chips and sparks: (a) EMBA 245 - Employees repositioned cardboard boards and removed bad cardboard boards from the extendo conveyor on the WSA compact top prefeeder, model WSA-TPC-2500X1400, with the machine on. The extendo conveyor's belts and pulleys had no guarding on the end where boards fell into the EMBA 245's tamper, exposing employees to getting caught in in-going nip points, on or about July 12, 2023. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
