The guard designed to cover the rotating tooling was missing, and the limit switch that detects if the guard is in place was bypassed with a piece of metal. Instance b) On or about August 22, 2023, the employer did not ensure that a part revolution clutch Rousselle Model 2 15-ton (Serial 25333) mechanical power press on the Nudo 3 line was adequately guarded. The moveable plastic shield that was used as guarding did not prevent employees from reaching around the guard to reach the point of operation. Instance c) On or about October 12, 2023, the employer did not ensure that a part revolution clutch Rousselle Model 10SS80 100-ton (Serial 30500) mechanical power press on the PC1 line was adequately guarded on the sides and rear. The sides of the press were not guarded, and the backside was only guarded with plastic shield that did not prevent employees from reaching the point of operations. Instance d) On or about August 9, 2023, the employer did not ensure that a part revolution clutch Hidaka Model FLH35 35-ton (Serial 58002) mechanical power press on the RF6 line was adequately guarded. The moveable plastic shield that was used as guarding did not prevent employees from reaching around the guard into the point of operations. Instance e) On or about September 15, 2023, the employer did not ensure that a part revolution clutch Rousselle Model 6 60-ton (Serial 16134) mechanical power press on the RF Nudo #1 line was adequately guarded. The moveable plastic shield that was used as guarding did not prevent employees from reaching around the guard into the point of operation. 29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: Instance a) On or about July 25, 2023, the employer failed to protect the employee from unexpected release of hazardous energy during set-up on the Cut-off Rousselle press on PC#1(6b-48 60-Ton Double Gap Frame Press S/N:22918). The employees were exposed to machine hazards associated with moving parts and pinch points from the unexpected start-up and energization of PC#1 while performing maintenance such as (but not limited to) replacement of the screw on the spring located beneath the bottom die. Instance b) On or about October 3, 2023, employees were exposed to machine hazards associated with moving parts and pinch points from the unexpected release of energy during die set-up on press brake #1(Model 08/2010 Serial no. 16648). Instance c) On or about July 19, 2023, employees were exposed to machine hazards associated with moving parts and pinch points from the unexpected release of energy during die set-up on Amada press brake #3(RG-80 80-Tonx994,4 'Bed CNC Hydraulic Press Brake, S/N 805987). Instance d) On or about July 19, 2023, employees were exposed to machine hazards associated with moving parts and pinch points from the unexpected release of energy during die set-up on Accurpress press brake #6 (Model:71008 100-Ton x8'Bed CNC Hydraulic Press Brake S/N:4918). Instance e) On or about October 12, 2023, employees were exposed to machine hazards associated with moving parts and pinch points from the unexpected energization of PC #3 line during the coil set-up process.
