(a) On July 12, 2023, at or near 16495 52nd Street SE Kindred, ND, the employees were using a platform attached to a JLG rough terrain forklift to raise and lower personnel to the eave of a pole barn, where the platform width extended more than 9 feet beyond the width of the forklift wheelbase.
