29 CFR 1910.119(f)(1)(ii): The employer did not develop and implement written operating procedures providing clear instruction for safely conducting activities in each covered process consistent with the process safety information addressing operating limits: On or about July 14, 2023, in the Glycol II unit, the Responding to Hi D-87 Level Procedure did not include a direct reference to a safety instrumented system in the Glycol II unit that automatically activated and functioned as designed to facilitate the emergency shutdown of equipment during the incident.
