Walking-working surfaces were not maintained free of hazards such as sharp or protruding objects and loose boards in that, on or about July 13, 2023, in the Maceration building of Azteca Milling LLC located at 1388 Co Rd U, Plainview, TX 79072, employees engaged in drain cleaning were exposed to tripping and falling hazards due to a loose and lifted metal cover that was not flush with surrounding walkway.
