Worksite: On or about July 17, 2023 the employer exposed employees to struck-by hazards, in that, the employer required employees to operate powered industrial trucks prior to the employees being trained in the safe operation of the equipment.

29 CFR1910.212(a)(1):  One or more methods of machine guarding was not provided to protect the operator and other employees from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks.  a) Worksite:  On or about July 17, 2023 printing press folding machine 5 was in use with unguarded drive belts and drive gears exposing employees to caught-in and amputation hazards in the press department.  b) Worksite:  On or about July 17, 2023 printing press folding machine 6 was in use with unguarded drive belts and drive gears exposing employees to caught-in and amputation hazards in the press department.  c) Worksite:  On or about July 17, 2023 binder/seaming machine 1 was in use with unguarded mechanical moving parts exposing employees to caught-in and amputation hazards in the post-press department.  d) Worksite:  On or about July 17, 2023 binder/seaming machine 3 was in use with unguarded mechanical moving parts exposing employees to caught-in and amputation hazards in the post-press department.

29 CFR1904.41(a)(2): Annual electronic submission of OSHA Form 300A Summary of Work-Related Injuries and Illnesses by establishments with 20 or more employees but fewer than 250 employees in designated industries. If your establishment had 20 or more employees but fewer than 250 employees at any time during the previous calendar year, and your establishment is classified in an industry listed in appendix A to subpart E of this part, then you must electronically submit information from OSHA Form 300A Summary of Work-Related Injuries and Illnesses to OSHA or OSHA's designee. You must submit the information once a year, no later than the date listed in paragraph (c) of this section of the year after the calendar year covered by the form.  Facility:  On or about July 17, 2023 the employer having 75 employees and operating under NAICS Code 323111 - Commercial Printing (except Screen and Books), had not electronically submitted their OSHA 300A Annual Summary data for calendar year 2022 which was required by March 2, 2023.
