29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles.   On or about July 17, 2023 the employer G L and E Construction LLC, at worksite located at 4055 Faith Dr. Huron OH 44839, did not ensure that each affected employee used appropriate eye or face protection when performing framing and sheeting work on a new build residential structure while exposed to potential struck by hazards (flying particles/debris) while using a DeWalt 7 ?" cordless circular saw (Model DCS575) and pneumatic nail guns.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about July 17, 2023 the employer G L and E Construction LLC, at worksite located at 4055 Faith Dr. Huron OH 44839, did not ensure the side rails of one portable Werner brand MT22 aluminum multi position ladder was extended at least three feet above the landing surface. Two employees were exposed to falls from approximately 11 feet or greater from the building eave to the ground below, while using the ladders to access the second (roof) level to perform residential construction activities on a new build residential structure.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about July 17, 2023 the employer G L and E Construction LLC, at worksite located at 4055 Faith Dr. Huron OH 44839, did not ensure employees were protected from falls while conducting residential framing construction activities on the second (roof) level of a residential single family home, while exposed to a fall hazard of approximately 20 feet and 7 tenths or greater to the ground below.  G L and E Construction LLC, was previously cited for a violation of this Occupational Safety and Health Standard 1926.501(b)(13), which was contained in OSHA inspection number 1516680, citation number one, item number one and was affirmed as a final order on October 27, 2021, with respect to a workplace located at 12914 Fancher Road, Lot 24, Westerville, OH 43081.
