29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) 19482 Washington Street, Watertown, NY  13601.  On the joists and partial decking of the building  during demolition.  On or about 7/14/23, an employee was working at 12 feet from grade, while demolishing a wood frame building from the joists down, and without fall protection of any kind.

29 CFR  1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level:  a) 19482 Washington Street, Watertown, NY  13601.  On the joists and partial decking of the building  during demolition.  On or about 7/14/23, employees were working at 12 feet from grade, along the edge of the platform, while demolishing a wood frame building from the joists down, where the location of the anchors and the length of the lifelines would ensure a greater than 6 foot free fall and likely striking the lower level.
