On the joists and partial decking of the building during demolition. On or about 7/14/23, an employee was working at 12 feet from grade, while demolishing a wood frame building from the joists down, and without fall protection of any kind.

29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level: a) 19482 Washington Street, Watertown, NY 13601. On the joists and partial decking of the building during demolition. On or about 7/14/23, employees were working at 12 feet from grade, along the edge of the platform, while demolishing a wood frame building from the joists down, where the location of the anchors and the length of the lifelines would ensure a greater than 6 foot free fall and likely striking the lower level.
