Employee was exposed to fall hazards. b) Worksite, 5th tier of scaffold, north-facing wall: On or about July 18, 2023, an employee conducting masonry work on the 5th tier of a fabricated frame scaffold that was not fully planked. Employee was exposed to fall hazards.

29 CFR 1926.451(e)(5)(i): Ramps and walkways 6 feet (1.8 m) or more above lower levels did not have guardrail systems that comply with subpart M of this part: a) Worksite, 6th tier of scaffold, ramp: On or about July 18, 2023, an employee was walking on a ramp between two scaffold frames on the 6th tier of a fabricated frame scaffold. The 6th tier was approximately 36 feet above the ground and the rap was not equipped with a guardrail system. Employee was exposed to fall hazards.
