29 CFR 1904.4(a): The employer did not record each work-related fatality, injury or illness case that resulted in the general recording criteria on the OSHA Form 300 or equivalent. Company: On, or about, July 18, 2023, and at times prior thereto, a Log of all Work-Related Injuries and Illnesses (OSHA Form 300) was not created by the establishment. On July 17, 2022, an employee suffered a severe head injury which required medical treatment beyond first aid and was hospitalized for an extended period of time. The injury was not recorded on the OSHA 300 form. Abatement Note: In addition to completing the OSHA Form 300, an OSHA 300A Summary form must be completed and posted per 29 CFR 1904.32. Abatement certification and documentation is required.
