(a) Warehouse: On or about 7/18/23, the employer did not document compliance with the chosen RAGAGEP such as IIAR bulletin 109 Section 4.4.2, when overhead air handling units and anhydrous ammonia piping were not protected from being struck by palletized ice and operating forklift trucks. (b) Machinery room -- On or about 7/18/23, the employer did not document compliance with the chosen RAGAGEP such as IIAR-2 Section 13.3.1.4, when the machinery room did not have an emergency eyewash and shower located external to the machinery room and readily accessible via an exit.

29 CFR 1910.119(j)(5):The employer did not correct deficiencies in equipment that were outside acceptable limits (defined by the process safety information on paragraph (d) of this section) in a safe and timely manner when necessary means were not taken to assure safe operation. (a) Condenser platform, exterior of plant -- On or about 7/18/23, the employer did not correct deficiencies in equipment that did not comply with the employer's chosen RAGAGEP such as IIAR Bulletin 109 Section 4.7.4, when anhydrous ammonia piping associated with the Frick condenser unit(s) had visible external coating failure and corrosion.
