29 CFR 1926.651(b)(1): The estimated location of underground utility installations, such as sewer, telephone, fuel, electric, water lines, or any other underground installations that reasonably may be expected to be encountered during excavation work, were not obtained prior to commencing operations: 17 St. James Place, Lewiston - The employer did not ensure that the employee operating an excavator were aware of the location of an underground electrical service line resulting in the employee hitting the buried line with the excavator bucket while moving a soil pile.
