29 CFR  1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  COMPANY-WIDE:  On or about July 19, 2023,the employer had not developed machine-specific Lockout/Tagout procedures for each of their machines including but not limited to lathes and presses.

29 CFR  1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips. The pressure machine is operated with one hand and exposes injure to the opposite hand.  Location: Downstairs Workshop.  On or about July 19, 2023, the employer did not ensure that the pressure machine had a guard for the operator. The guard had an exposing point with a single lever use. The operator could move the lever with one hand and use his other to put/remove metal objects exposing them to the hazard.

29 CFR  1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle:  COMPANY-WIDE: On or about July 19, 2023, the point of operation of the Lathe Milling Machine was unguarded thereby exposing the operator of the machine to in-running nip points.

29 CFR  1910.134(k)(6): The employer did not provide the basic advisory information on respirators, as presented in Appendix D of 29 CFR 1910.134, in written or oral format to employees who wear respirators when such use was not required by the employer: Employees wear mask and employer supplies  COMPANY-WIDE:   On or about July 19, 2023, employees voluntarily wore N-95 respirators when working with metals including but not limited to aluminum, stainless steel, brass, and copper.  The employer provided the N-95s but did not provide employees with Appendix D training.
