(a) At 5351 SR 69, Cullman, AL: On or about 7/12/2023, the employer exposed employees to contact with electrical source in that, equipment, such as but not limited to, Hastings 8' External Rod Shotgun Stick Model 8108-U had not been tested and certified for the worksite where employees were performing tasks, such as but not limited to, connecting underground secondary to a vertical pole mounted switch near three phase 14,400 V energized primary.
