29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c). On or about July 19, 2023, employees were observed working in a trench and used a ladder located outside of the trench box to ingress and egress the trench box. The trench was more than 5 feet in depth and exposed employees to the hazard of falling/collapsing soil.
