29 CFR 1904.39(a)(2):The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. At the workplace located at 25102 Holt 250 Rd Forest City, Missouri 64451, the employer did not report a hospitalization as a result of a work-related incident to OSHA within twenty-four (24) hours. An employee hospitalization occurred on or about July 11, 2023. The hospitalization was not reported to OSHA until July 14, 2023.
