29 CFR 1910.140(c)(13)(ii): Anchorages were not designed, installed, and used, under the supervision of qualified person, as part of a complete personal fall protection system that maintains a safety factor of at least two: (a) On or about 7/20/2023, at 205 Highpoint Dr, Venice, FL, employees were exposed to fall hazards of approximately 30 feet while descending from a tree that had the top removed, in that, an inadequate anchor was installed.
