29 CFR 1910.179(n)(3)(i): The load was not well secured and properly balanced in the sling or lifting device before it was lifted more than a few inches: Employees engaged in material handling activities were exposed to struck-by hazards in that employer did not ensure that the load of steel tubing was well secured, correctly setup, and properly balanced to protect the operators and other surrounding employees in the event of a falling load due to inadvertent failure of the crane mechanism or operator error.
