On or about July 14, 2023, and at times prior thereto, flexible cords were running through the ceilings to energize machines exposing employees to electrical and fire hazards in the following instances: a. Flexible cord protruding out of holes in packing area ceiling powering the filler machine #2, metal detector, and packing machine; b. Flexible cord protruding out of holes in the blending room to energize the blender and mixer machines.

29 CFR 1910.157(c)(1): Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries. On or about July 14, 2023, on the north side of the packing room, the employer did not ensure employees had clear access to the fire extinguisher at all times. Employees were exposed to fire and smoke hazards.
