29 CFR  1910.134(i)(7): Oil-lubricated compressor(s) equipped with only high-temperature alarm(s) were used to supply breathing air and the air supply was not monitored at intervals sufficient to prevent carbon monoxide in the breathing air from exceeding 10 ppm:  On or about July 26, 2023, an Atlas Copco G11-FF air compressor, equipped with only a high-temperature alarm, supplied breathing air to glass-room employees' supplied air respirators  that was not monitored at intervals sufficient to prevent carbon monoxide in the breathing air from exceeding 10 ppm.

29 CFR  1910.1000(d)(1)(i): When determined by the computational formula specified in 29 CFR 1910.1000(d)(2)(i), employees were exposed to concentrations of more than one substance for which 8-hour time weighted averages are listed in Subparts Z of 29 CFR part 1910 in excess of the permissible exposure for the mixture:  On July 26, 2023, the employer did not ensure that employee exposure to concentrations of more than one substance did not exceed the permissible exposure limit for the mixture of heavy metals to include inorganic Lead and Cadmium. This mixture has additive toxic effects on the kidneys. An employee dismantling Cathode Ray Tubes in the glass room in the CRT area was exposed to inorganic Lead at an 8-hour time weighted average (TWA) of 0.0523 milligrams per cubic meter (mg/m3) of air, approximately 1.046 times the Permissible Exposure Limit (PEL) of 0.05 mg/m3. The employee was also exposed to Cadmium at an 8-hour TWA of .0021 mg/m3 of air, approximately 0.42 times the PEL of 0.005 mg/m3. The exposure to the mixture was approximately 1.46 times the permissible 8-hour TWA concentration for the mixture. Sampling was performed for 416 minutes, with zero exposure assumed for the 64 minutes not sampled.

29 CFR  1910.1025(d)(6)(ii):  Where the initial determination or subsequent monitoring revealed employee exposure to lead to be at or above the action level but below the permissible exposure limit; monitoring was not repeated at least every six months:  On July 26, 2023, in the glass room in the CRT area of the workplace located at N5549 County Road Z, Onalaska, WI, employees manually breaking the leaded glass of Cathode Ray Tubes and exposed to lead above the action level had not had their personal exposure to lead monitored at least every six months.

Recycling Production 1 employees shoveled and swept lead accumulations in the glass room and the dropout of the funnel glass pulverisor.
