29 CFR 1926.451(b)(5)(ii): The end of a platform greater than 10 feet in length extended over its support more than 18 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees without tipping, or has guardrails which block employee access to the cantilevered end: a. On or about July 17, 2023, at the above addressed jobsite, an employee was exposed to falls while performing siding operations from a pump jack scaffold, approximately twelve (12) feet ten (10) inches above the ground, due to the walking platform exceeding 18 inches from the support bracket.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. a. On or about July 17, 2023, at the above addressed jobsite, an employee conducting siding installation operations from a ladder jack scaffold, twelve (12) feet seven (7) inches above the ground, was not provided with an access ladder, thereby exposing the employee to falls.
