29 CFR 1910.140(c)(18): The employer did not ensure that personal fall protection systems are inspected before initial use during each work shift for mildew, wear, damage, and other deterioration, and defective components are removed from service. On or about July 20, 2023, an employee used a personal fall protection system to mount a truck tanker to clean tank without inspecting the body harness. The leg strap's tongue buckles on the body harness displayed deterioration and rust exposing the employee to a fall hazard.
