29 CFR  1910.147(c)(1): Energy control program.  The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative.  a) Production Area On or about July 19, 2023, the employer failed to an establish energy control program, which would include procedures, training, and periodic inspections to protect employees from hazards associated with unexpected startup and energy release from equipment when performing servicing and maintenance activities.  Employees were exposed to caught-in and amputation hazards when employees performed maintenance and set-up activities on the Staley (MN: 66"x 112") Rotary Die Cutter (SN J6200).  b) Production Area On or about July 19, 2023, the employer failed to an establish energy control program, which would include procedures, training, and periodic inspections to protect employees from hazards associated with unexpected startup and energy release from equipment when performing servicing and maintenance activities. 29 CFR  1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks.  Examples of guarding methods are barrier guards, two-hand tripping devices, electronic safety devices, etc.  a) Production Area  On or about July 17, 2023, employees working on the Staley (MN: 66"x 112") Rotary Die Cutter (SN J6200) were exposed to amputation and laceration hazards when operating machine.  There was an unguarded belt pulley near a lock-out point of machine exposing employees to ingoing nip point hazards.  The employer did not adequately guard the machine to prevent employees from getting their hands / fingers caught in the moving parts of the machine.      b) Production Area  On or about July 17, 2023, employees working on the Langston (MN: 5113 Saturn 3) Rotary Die Cutter (SN: 957585) were exposed to amputation and laceration hazards during normal production.  The interlocked gate was not functional, and no other effective protection was in place. The employer did not adequately guard the machine to prevent employees from getting their hands/fingers caught in the moving parts of the machine.     Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

Pulleys, any parts of which are seven (7) feet or less from the floor or working platform, shall be guarded in accordance with the standards specified in paragraphs (m) and (o) of this section.   a) On or about July 17, 2023, employees in the production area were exposed to an unguarded belt pulley while operating the Staley (MN: 66"x 112") Rotary Die Cutter (SN J6200).   Employees were exposed to amputation and laceration hazards.  The employer did not adequately guard the machine to prevent employees from getting their hands / fingers caught in the moving parts of the machine.

29 CFR  1910.178(l)(6):  Certification. The employer shall certify that each operator has been trained and evaluated as required by this paragraph (l). The certification shall include the name of the operator, the date of the training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation.  a) On or about July 19, 2023, the employer did not ensure that employees who were authorized to operate powered industrial vehicles were certified.  The employees had not been trained and evaluated as required.    Abatement documentation is required for this item in accordance with 29 CFR 1903.23(d)
