Employee(s) engaged in core cutting activities using the 7-inch Craftsman horizontal band saw were exposed to amputation and struck by hazards in that the unused portion of the blade was not guarded. Approximately 12 inches of unused portion of the blade was exposed.

29 CFR 1910.253(b)(4)(iii): Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one-half hour Employees working near foil line work station 10 were exposed to fire and explosion hazards. The employer did not ensure that oxygen cylinders were stored separately from highly combustible materials. Oxygen cylinders were being stored immediately adjacent to acetylene and oxygen cylinders.
