On or about July 21, 2023, at the above addressed jobsite, employees were not trained in the recognition of jobsite hazards, which included, but were not limited to, the hazards associated with falls while conducting roofing operations.

29 CFR 1926.501(b)(13):  Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).  a.  On or about July 21, 2023, at the above addressed jobsite, employees performing residential roofing operations, ten (10) feet six (6) inches above the ground, were not protected from falls, thereby exposing employees to fall hazards.  JM Exteriors, Inc. was previously cited for a violation of this Occupational Safety and Health standard 1926.501(b)(13), which was contained in OSHA inspection number 1483882, citation number 1, item number 1 and was affirmed as a final order on November 12, 2020, with respect to a workplace located at 10845, 10847 Canterbury Drive, Mokena, IL 60448.  JM Exteriors, Inc. was previously cited for a violation of this Occupational Safety and Health standard 1926.501(b)(13), which was contained in OSHA inspection number 1408059, citation number 1, item number 1a and was affirmed as a final order on October 1, 2019, with respect to a workplace located at 19351-19361 Lisadell Drive, Tinley Park, IL 60487.
