On or about July 20, 2023, Adrian Drive, Deerfield Estates in Sewickley, PA 15143:  Employees were exposed to fall hazards in excess of 20 feet to the ground surface below while performing siding installation along with other related activities from a pump jack's walking and working surface that extended in excess of 4 feet over the support.

On or about July 20, 2023, Adrian Drive, Deerfield Estates in Sewickley, PA 15143: Employees working from scaffolding to install siding on a new residence were not protected by the use of fall protection. An unprotected employee fell 18 feet and incurred fatal injury.
