29 CFR 1910.333(c)(3)(iii)(A): Any vehicle or mechanical equipment capable of having parts of its structure elevated near energized overhead lines was not operated so that a clearance of 10 ft. (305 cm) is maintained. On or about July 20, 2023, employees operating and maintaining a Viper Belt sand conveyor were exposed to electrical shock hazards when the minimum required clearances were not maintained from the energized overhead power lines.

29 CFR 1910.335(b)(1): Safety signs and tags. Safety signs, safety symbols, or accident prevention tags shall be used where necessary to warn employees about electrical hazards which may endanger them, as required by 1910.145. On or times prior to July 20, 2023, employer failed to provide adequate warning signs to alert employees of high voltage overhead powerlines, exposing employees to electric shock and electrocution hazards.
