On or about April 12, 2023, an employee was elevated approximately 12 feet above the concrete floor on a pallet placed on the forks of a Clark Model ECS25 forklift in order to access wiring and install a camera system inside the building near the training room. The un-approved work platform exposed the elevated employee to fall hazards.
