29 CFR 1910.243(c)(3): Abrasive wheels used on right angle head grinders were not provided with safety guard(s) having a maximum exposure angle of 180 degrees and located between the operator and wheel: Road Crew shop: A Metabo portable right angle grinder with 4.5 inch abrasive grinding wheel attachment was missing the abrasive wheel guard. Employees were exposed to face and hand lacerations.

29 CFR 1910.212(b): Machine(s) designed for fixed location(s) were not securely anchored to prevent walking or moving: Warehouse, Engine and Hand Crew area: An ACE Hardware 6-inch bench mounted grinder was not anchored to the work bench to prevent it from tipping while in use. Employees were exposed to crushing injuries. The USDA, Forest Service was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(b), which was contained in OSHA inspection number1547228, citation number 1, item number 1b and was affirmed as a final order on 10/15/2021, with respect to a workplace located at 18050 Mulock Rd., CHALLENGE, CA 95925 The USDA, Forest Service was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(b), which was contained in OSHA inspection number1481082, citation number 1, item number 1 and was affirmed as a final order on 10/21/2020, with respect to a workplace located at 44630 Hwy. 96, SEIAD VALLEY, CA 96086
