Employees were observed operating the machine with guards open while troubleshooting the equipment or performing setup. Hazardous energies present on the equipment include but are not limited to electrical, pneumatic, and mechanical.

Employees are tasked with service and maintenance including setup on equipment including but not limited to shavers, headers, horizontal drilling machines, and tapping machine. Hazardous energies present on equipment include but are not limited to mechanical, electrical, pneumatic, and hydraulic.

29 CFR 1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) On or about July 24, 2023, the employer did not ensure that effective machine guarding was in place on the #3 shaver while in production to protect employees from the moving machine components of the equipment. Plexiglass barrier guards were not in place or closed to afford protection to operators. b) On or about July 24, 2023, the employer did not ensure that effective machine guarding was in place on the #2 shaver while in production to protect employees from the moving machine components of the equipment. Plexiglass barrier guards were not in place or closed to afford protection to operators. Additionally the fixed barrier guard covering the belt and pulley nip point of the motor was not in place. c) On or about July 24, 2023, the employer did not ensure that effective machine guarding was in place on the #1 shaver while in production to protect employees from the moving machine components of the equipment. Plexiglass barrier guards were not in place or closed to afford protection to operators. Additionally the fixed barrier guard covering the belt and pulley nip point of the motor was not in place. d) On or about July 24, 2023, the employer did not ensure that effective machine guarding was in place on the Waterbury Header. Employees operate the equipment without the presence of any effective guarding to prevent them from reaching into the moving machine components including the progressive die area.
