29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:   On or about February 22, 2024, the employer did not ensure machine specific procedures were utilized exposing employees to a caught-in hazard while completing maintenance to snow machine #10 in the production area. Hazardous energies were still present on snow machine #10 when the pneumatic clamps were still engaged as an employee attempted to clear a malfunction, resulting in an avulsion.   Sherex Fastening Solutions was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.147(c)(4)(i), which was contained in OSHA inspection number 1685470, citation number1, item number1 and was affirmed as a final order on 1/25/2024, with respect to a workplace located at 850 Moe Drive Akron, Ohio 44310.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a) On or about March 19, 2024, the employer did not ensure machine guarding was in place on snow machine #1 exposing employees to a caught-in/amputation hazard. Employees operate the snow machine without the presence of any effective machine guarding in place to prevent them from reaching into the active drill bit and pneumatic powered fastener clamps during fastener production.   b)  On or about March 19, 2024, the employer did not ensure machine guarding was in place on snow machine #2 exposing employees to a caught-in/amputation hazard. Employees operate the snow machine without the presence of any effective machine guarding in place to prevent them from reaching into the active drill bit and pneumatic powered fastener clamps during fastener production.   c) On or about March 19, 2024, the employer did not ensure machine guarding was in place on snow machine #3 exposing employees to a caught-in/amputation hazard. Employees operate the snow machine without the presence of any effective machine guarding in place to prevent them from reaching into the active drill bit and pneumatic powered fastener clamps during fastener production.   d) On or about March 19, 2024, the employer did not ensure machine guarding was in place on snow machine #4 exposing employees to a caught-in/amputation hazard. Employees operate the snow machine without the presence of any effective machine guarding in place to prevent them from reaching into the active drill bit and pneumatic powered fastener clamps during fastener production.   e) On or about March 19, 2024, the employer did not ensure machine guarding was in place on snow machine #5 exposing employees to a caught-in/amputation hazard. Employees operate the snow machine without the presence of any effective machine guarding in place to prevent them from reaching into the active drill bit and pneumatic powered fastener clamps during fastener production.   f) On or about March 19, 2024, the employer did not ensure machine guarding was in place on snow machine #6 exposing employees to a caught-in/amputation hazard. Employees operate the snow machine without the presence of any effective machine guarding in place to prevent them from reaching into the active drill bit and pneumatic powered fastener clamps during fastener production.   g) On or about March 19, 2024, the employer did not ensure machine guarding was in place on snow machine #7 exposing employees to a caught-in/amputation hazard. Employees operate the snow machine without the presence of any effective machine guarding in place to prevent them from reaching into the active drill bit and pneumatic powered fastener clamps during fastener production.   h) On or about March 19, 2024, the employer did not ensure machine guarding was in place on snow machine #8 exposing employees to a caught-in/amputation hazard. Employees operate the snow machine without the presence of any effective machine guarding in place to prevent them from reaching into the active drill bit and pneumatic powered fastener clamps during fastener production.   i) On or about March 19, 2024, the employer did not ensure machine guarding was in place on snow machine #9 exposing employees to a caught-in/amputation hazard. Employees operate the snow machine without the presence of any effective machine guarding in place to prevent them from reaching into the active drill bit and pneumatic powered fastener clamps during fastener production.   j) On or about March 19, 2024, the employer did not ensure machine guarding was in place on snow machine #10 exposing employees to a caught-in/amputation hazard. Employees operate the snow machine without the presence of any effective machine guarding in place to prevent them from reaching into the active drill bit and pneumatic powered fastener clamps during fastener production.   k) On or about March 19, 2024, the employer did not ensure machine guarding was in place on snow machine #11 exposing employees to a caught-in/amputation hazard. Employees operate the snow machine without the presence of any effective machine guarding in place to prevent them from reaching into the active drill bit and pneumatic powered fastener clamps during fastener production.
