29 CFR 1910.178(a)(4):Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: Location - Middle of the yard/tree staging area On or about 7/20/2023, employees were utilizing a Kubota R540 Tractor with fork attachments installed that did not have the manufacturer's written approval to use it with their equipment, exposing employees to incidents while elevating an employee with the forks/man-basket.
